Benefits - Health Innovations shall provide the consumers enrolled in the HI membership program access to our wide array of health related discounts. Including but not limited too: prescriptions, dentist visits, hearing and ear care, and a 24 nurseline. For a complete list of the current benefit discount offerings please check the HI website at www.health-innovations.com. Health Innovations reserves the right to add or remove discount providers at anytime from the HI program.

Coverage - Your Health Innovations membership covers you, your spouse and any dependants living in your household. Your Health Innovations membership also includes a subscription to :Life Extension magazine. There are no obligations and no hidden charges. The cost of your subscription is included in your membership. If you decide that you do not want this magazine subscription and would like a refund for the value of a one year subscription simply write your membership number on a copy of this page and send it with a self addressed stamped envelope within 30 days of joining, to ValueMags, 212 Superior St., Ste. 300, Chicago, IL 60610 and you will be refunded the $8.99 subscription fee.

Membership Billing and Renewals - For your convenience and to provide you with uninterrupted benefit coverage, unless you call to cancel, your membership will be automatically renewed each billing term, charged the then current term fee, to the credit card that was originally provided during enrollment.

Cancellation and Refunds - If you are an annual bill term member and after your 1-month introductory period if you're ever dissatisfied with your Health Innovations benefits, simply call 1-800-856-4002 toll free to cancel and receive a refund for the unused portion of that year's annual membership fee minus a small cancellation fee of $5. If you are a monthly bill member simply call the number listed above, toll free, to cancel and owe nothing further. If you cancel during your trial, we keep the $1 but you owe nothing further. And the incentive/premium is yours to keep no matter what, however you must be an active member at the time of redemption. Liability - Consumer Innovations shall assume no responsibility and shall have no liability to a consumer's use of any benefit in the HI membership program. Consumers enrolled in the HI program that have any problem with their use of the benefits of the HI program agree to take action against the provider of that benefit and not Consumer Innovations.

Governing Law; Arbitration - This agreement shall be governed and construed in accordance with the laws of the state of Arizona. Any dispute arising between HI and a consumer enrolled in HI will be submitted to arbitration in the State of Arizona in accordance with the rules of the American Arbitration Association.
